Pemberton/Whistler Karate Club - Membership Application


Please provide the following information:












Membership Conditions


  1. Member agrees to conduct him/herself in such a manner that is proper and to respect all Dojo Health Rules and Regulations.
  2. Any flagrant violation of Dojo Health Rules may result in having membership suspended with no refund.
  3. The club is not responsible for accidents or loss of personal property.
  4. The member hereby stipulates that he or she is physically sound and that he or she has medical approval to proceed with a normal routine or exercise.

    5. LIMITATIONS, IF ANY, ARE NOTED BELOW:

It is further expressly agreed that all exercises shall be undertaken by me, the undersigned, at my sole risk and that the Company, Corporation, Dojo or Club shall not be liable to me for any claims, demands, injuries, damages actions or causes of action, whatsoever, to my person or property arising out of or connected with the use by me of the services and facilities of such Company, Corporation, Dojo or Club or the premises where the same are located and I do hereby expressly forever release and discharge the said Dojo, Company, Corporation or Club from all such claims, demands, injuries, damages, actions, or causes of action, and from all acts of active or passive negligence on the part of such Company, Corporation, Dojo or Club, it's Servants, Agents or Employees.
  6. I, the undersigned, promise to pay to the Pemberton/Whistler Karate Club, the fees as determined by the executive of the club.
  7. If for any reason I, the undersigned, am unable to, or do not take advantage of the benefits offered, or use the facilities of the Dojo, I am in no way relieved of the obligation and payments and no refunds will be made.

    8. Applicant has read this contract and accepts all the conditions of membership without any reservations whatsoever.
